Growth & Outreach Coordinator
Primary Responsibilities (including, but not limited to):
Identify key local referral sources and community partners , including hospitals, nursing homes, assisted living facilities, rehabilitation centers, physician offices, and other healthcare providers.
Build, maintain, and nurture a database of current and potential referral partners.
Coordinate and conduct educational outreach and community presentations focused on senior care, rehabilitation services, and the unique offerings of Friendly Nurses Home Health Care.
Develop relationships with individuals and organizations in a position to refer patients to home health services, including private, public, and non-profit sectors.
Arrange and deliver presentations of Friendly Nurses Home Health Care services to community organizations, healthcare professionals, and relevant businesses.
Maintain consistent outreach presence through in-person meetings, phone calls, and digital engagement with community partners.
Collaborate with internal team members to ensure high-quality follow-through on referrals and community relationships.
Maintain and manage an effective Customer Relationship Management (CRM) system to track outreach activities, leads, and follow-up actions.
Use internal tracking tools to document outreach activities and submit reports to management on a weekly basis.
Participate in weekly meetings with management to review progress, opportunities, and strategic outreach initiatives.
Experience:
Healthcare or home health experience preferred — understanding of healthcare landscape is essential.
Proven experience in outreach, community engagement, or business development .
Strong project management skills, with the ability to manage multiple community relationships and initiatives simultaneously.
Excellent communication and interpersonal skills, with the ability to build trust and rapport with healthcare professionals and community partners.
Proficient in Customer Relationship Management (CRM) software and basic reporting tools.
Strong organizational skills with an emphasis on relationship-building over transactional sales .
